WASHINGTON — In 2026, six childhood vaccines were moved from "routine" to "shared clinical decision-making": rotavirus, COVID-19, influenza, hepatitis A, hepatitis B, and meningococcal. HPV was reduced from 2-3 doses to one. That bureaucratic phrase — "shared clinical decision-making" — means one thing in practice: your insurance may no longer cover them.

Out-of-pocket costs without insurance in 2026: Hepatitis B series, $120-$330. Hepatitis A series, $170-$230. Meningococcal, $170-$280. COVID-19, $90-$200 per dose. A family completing the full previously-recommended childhood schedule out of pocket faces well over $1,000.

The Vaccines for Children program — which provides free vaccines to uninsured and underinsured kids — only covers CDC/ACIP-recommended vaccines. When vaccines lose their routine recommendation, they automatically fall out of VFC coverage. The children who most need free access lose it first.

The Dispatch

The phrase "shared clinical decision-making" sounds like empowerment. It is a billing code.

When the government says it is giving parents a choice, what it means is: parents who can afford the vaccine will get it. Parents who cannot will not. The "choice" is between paying and going without.

First Focus on Children said it plainly: "Only children whose families can afford them will be vaccinated, further exacerbating the economic disparities in the U.S. health care system." That is not a side effect. That is the policy.

The same administration that cut vaccine recommendations also proposed cutting the CDC's budget. They are not pro-choice on vaccines. They are pro-cost.
